ABOUT LONGITUDE PHM

Longitude PHM (population health management) will focus on optimizing care delivery for Medicare Advantage, Medicare Shared Savings Program, and other ACO beneficiaries, empowering provider organizations to improve the efficiency, utility and seamlessness of care delivery in local communities.

As health care progressively shifts from a fee-for-volume model to one focused on value, Longitude PHM will play a key role in helping health systems unleash the potential of managing populations more comprehensively, forge innovations in care delivery, and drive adoption of enabling solutions and technologies.

It is a challenging environment for health care providers to navigate the shift from fee-for-volume to a model focused on values.

Longitude PHM aims to improve performance of health systems engaging in population health and bring solutions to market that make care delivery more accessible and approachable.

The current and future member systems of Longitude PHM will be involved in the development of the diverse solutions that will involve partnering, integrating, and building strategies.

The immediate approach for Longitude PHM includes:

  • Bringing health systems into the discussion of shifting the value in care delivery
  • Identifying what effectively contributes to achieving population health goals and what does not
  • Developing solutions that are molded to meet the unique needs of individual health systems, and support both primary care and specialty clinicians in their transition to value
  • Implementing solutions in health systems for delivering quality care more affordably

Longitude PHM is expected to launch in early 2025.

Prominent physician leader draws on experience as a health system and
health plan executive to enhance care coordination and create value

[DEC. 10, 2024] – Longitude Health has named Craig Samitt, MD, as chief executive officer of Longitude PHM, a next-generation value-based care enablement company. Samitt will develop, lead and accelerate the effort to improve performance of health systems engaging in population health, while bringing solutions to market that make care delivery more accessible and approachable.

Longitude PHM (population health management) will focus on optimizing care delivery for Medicare Advantage, Medicare Shared Savings Program, and other ACO beneficiaries, empowering provider organizations to improve the efficiency, utility and seamlessness of care delivery in local communities. As health care progressively shifts from a fee-for-volume model to one focused on value, Longitude PHM will play a key role in helping health systems unleash the potential of managing populations more comprehensively, forge innovations in care delivery, and drive adoption of enabling solutions and technologies.

“There are significant, untapped opportunities in value-based care. Many health care organizations are still challenged today in effectively managing risk and performing well in broader population health contracts. Longitude PHM will provide health systems with the capabilities to be the central and essential players in the evolving value-based care landscape,” said Paul Mango, chief executive officer of Longitude Health. “Achieving this goal will require confronting the barriers health systems have faced when attempting to engage meaningfully in population health. Craig Samitt has the demonstrated expertise, experience and uncommon ability to identify and eliminate such barriers.”

Longitude PHM will provide solutions that can be modifiable to meet the unique needs of individual health systems and support providers across various points in the value continuum. The member systems of Longitude PHM will be involved in the development of the diverse solutions that will involve partnering, integrating and building strategies.

A nationally recognized expert and thought leader in industry transformation, care delivery and health care policy, Samitt has built bridges across multiple sectors in the health care system to develop and advance solutions that enhance care quality and reduce costs. With Longitude PHM, Samitt will continue his more than 30-year commitment to advocating for value-based care, improving operations, and empowering clinicians and health systems.

Samitt is the former president and CEO of Blue Cross and Blue Shield of Minnesota and previously served as EVP, chief clinical officer, and president of Anthem’s Diversified Business Group. An internal medicine physician by training, his career has spanned several senior executive positions, including partner and global provider practice leader at Oliver Wyman; president and CEO of HealthCare Partners, a subsidiary of DaVita HealthCare; and president and CEO of Dean Health System, one of the largest integrated health systems in the Midwest. Samitt holds an undergraduate degree from Tufts University, a medical degree from Columbia University and an MBA from the Wharton School of Business. He completed a medical residency in internal medicine at Brigham and Women’s Hospital in Boston and is a fellow of the American College of Physicians.

The announcement marks the second initiative for Longitude Health, building on the introduction of its first initiative, Longitude Rx, last month.
